Rasburicase is a medicine used to prevent and treat a condition called tumour lysis syndrome, which can occur in people receiving chemotherapy for certain cancers, especially blood cancers such as leukaemia and lymphoma. Tumour lysis syndrome happens when large numbers of cancer cells break down rapidly, releasing their contents into the bloodstream. This can lead to dangerously high levels of uric acid, potassium, and phosphate, which may cause kidney failure, heart rhythm problems, or other serious complications.
Rasburicase works by breaking down uric acid in the blood into a harmless substance called allantoin, which is easily excreted in the urine. Humans normally cannot break down uric acid efficiently, so rasburicase provides an enzyme that the body naturally lacks. By rapidly lowering uric acid levels, rasburicase helps protect the kidneys and prevents complications associated with tumour lysis syndrome.
Rasburicase is given by intravenous infusion, usually in a hospital setting. It is commonly administered once daily for a short period, often one to five days, depending on the patient?s risk level and response to treatment. Because of the risk of serious reactions, rasburicase is always given under close medical supervision with careful monitoring.
Rasburicase acts very quickly, often reducing uric acid levels within hours of administration. This rapid action makes it especially useful in high-risk patients who already have elevated uric acid levels or who are about to start intensive chemotherapy.
Common side effects of rasburicase include fever, headache, nausea, vomiting, diarrhoea, and mild allergic reactions such as rash or itching. These effects are usually mild and temporary. More serious allergic reactions can occur, including difficulty breathing, chest tightness, or swelling of the face or throat. These reactions require immediate medical attention.
A rare but serious side effect of rasburicase is haemolysis, which is the breakdown of red blood cells, especially in people with a condition called glucose-6-phosphate dehydrogenase deficiency. For this reason, patients are usually tested for this enzyme deficiency before receiving rasburicase. Rasburicase must not be used in individuals with known G6PD deficiency.
Rasburicase can also cause methemoglobinemia, a condition where oxygen delivery to tissues is impaired. Symptoms may include bluish discoloration of the skin or lips, shortness of breath, fatigue, or dizziness. This is rare but requires urgent medical attention.
During treatment, blood tests are regularly performed to monitor uric acid levels, kidney function, electrolytes, and blood counts. Patients are also monitored for signs of allergic reactions or infusion-related reactions.
Rasburicase is not used for long-term management of high uric acid levels. It is specifically used in emergency or high-risk situations related to cancer treatment. After the risk period has passed, other medications such as allopurinol may be used for ongoing control of uric acid.
When used appropriately, rasburicase is a highly effective and life-saving medication that prevents serious complications of tumour lysis syndrome. Its rapid action and targeted effect make it an essential treatment in oncology settings, especially for patients at high risk of metabolic complications during cancer therapy.
