Pemetrexed is a chemotherapy medicine used mainly to treat certain lung cancers, particularly non-small cell lung cancer, and a cancer called malignant pleural mesothelioma. Pemetrexed works by blocking several enzymes that cancer cells need to make DNA and RNA. These enzymes are essential for cell growth and division. By inhibiting these pathways, pemetrexed slows down or stops cancer cells from multiplying, which helps control tumour growth. Because cancer cells divide more rapidly than most normal cells, they are more affected by pemetrexed, although some healthy cells are also affected, which explains many of the side effects.
Pemetrexed is given by intravenous infusion into a vein. Pemetrexed is administered in a hospital or chemotherapy clinic by trained healthcare professionals. You will not take pemetrexed at home. The infusion usually takes about 10 minutes, but you may spend longer at the clinic for monitoring and other medicines. Pemetrexed is usually given once every three weeks, depending on your treatment plan.
Before and during pemetrexed treatment, you will be given additional medicines to reduce side effects. This is very important.
You will be prescribed folic acid tablets to take by mouth starting before pemetrexed and continuing during and after treatment. You will also receive vitamin B12 injections at regular intervals. Folic acid and vitamin B12 significantly reduce the risk of serious side effects from pemetrexed, especially effects on the blood and digestive system. It is essential to take these supplements exactly as instructed.
You will also usually be given a corticosteroid, such as dexamethasone, for a few days around each pemetrexed dose. This helps reduce the risk of skin rash and inflammation.

One of the most important effects of pemetrexed is lowering of blood counts. Pemetrexed can reduce white blood cells, red blood cells, and platelets. Low white blood cells increase infection risk, low red blood cells can cause fatigue and shortness of breath, and low platelets increase bruising and bleeding risk. Blood tests are done before each cycle. Seek urgent medical advice if you develop fever, chills, sore throat, unusual bruising, bleeding, or extreme tiredness.
Fatigue is very common with pemetrexed. This may build up over successive cycles. Rest when needed and pace your activities.
Nausea and vomiting can occur with pemetrexed, but these are often mild to moderate and usually controlled with anti-nausea medicines. Inform your care team if nausea is not controlled.
Pemetrexed can cause mouth sores, sore throat, or inflammation of the mouth and digestive tract. Good oral hygiene is important. Report mouth pain, ulcers, or difficulty swallowing early.
Skin rash can occur with pemetrexed, which is why steroids are given. Report any rash, itching, or peeling skin, especially if severe.
Pemetrexed can affect kidney function. Kidney function is checked with blood tests before each dose. Stay well hydrated unless advised otherwise. Report reduced urine output, swelling, or sudden weight gain.

Pemetrexed increases infection risk. Avoid close contact with people who are unwell and practise good hand hygiene. Any fever should be treated as urgent during chemotherapy.
Pemetrexed should not be used during pregnancy, as it can harm an unborn baby. Effective contraception is required during pemetrexed treatment and for the period advised after the last dose. Breastfeeding is not recommended during pemetrexed therapy.
