Nilotinib is a prescription medicine used to treat chronic myeloid leukaemia (CML) in adults and children who have a specific genetic abnormality called the Philadelphia chromosome. This abnormality causes the body to produce an enzyme known as BCR-ABL tyrosine kinase, which leads to uncontrolled growth of white blood cells. Nilotinib works by blocking this enzyme, helping to slow or stop the growth of cancer cells and restore more normal blood cell production.
Nilotinib belongs to a group of medicines called tyrosine kinase inhibitors. It is designed to be more selective and potent than some older treatments, making it effective even in some patients who do not respond well to earlier therapies.
Nilotinib is taken by mouth, usually twice daily, about 12 hours apart. It must be taken on an empty stomach, meaning no food for at least 2 hours before and 1 hour after each dose. Taking nilotinib with food can significantly increase drug levels in the body and raise the risk of serious side effects. The capsules should be swallowed whole with water and not opened, crushed, or chewed.
It is very important to take nilotinib exactly as prescribed and at the same times each day. Missing doses or taking extra doses can reduce effectiveness or increase side effects. If a dose is missed, it should not be doubled; the next dose should be taken at the regular time.
One of the most important safety concerns with nilotinib is its effect on the heart rhythm. It can cause a condition called QT prolongation, which can lead to serious and potentially life-threatening heart rhythm problems. Because of this, ECGs and blood tests are done before starting treatment and regularly during therapy. Low potassium or magnesium levels can increase this risk, so these electrolytes are monitored closely.
Nilotinib can also affect blood cell counts, causing low levels of white blood cells, red blood cells, or platelets. This can increase the risk of infections, fatigue, or bleeding. Regular blood tests are required to monitor these effects.
Common side effects include headache, nausea, vomiting, fatigue, muscle or joint pain, rash, itching, constipation, and abdominal discomfort. These are usually mild to moderate and often improve as the body adjusts to treatment.
Nilotinib may also affect the liver and pancreas, so liver enzymes and pancreatic enzymes are monitored regularly. Symptoms such as yellowing of the eyes or skin, severe abdominal pain, nausea, or dark urine should be reported immediately.
Some patients may experience increases in blood sugar or cholesterol levels. Long-term treatment may increase the risk of cardiovascular problems, so heart health is closely monitored, especially in people with existing risk factors such as diabetes, high blood pressure, or high cholesterol.
Nilotinib can interact with many other medicines, including certain antibiotics, antifungals, heart medications, and herbal products such as St John?s wort. Grapefruit and grapefruit juice should be avoided as they can increase nilotinib levels and raise the risk of side effects.
Nilotinib can harm an unborn baby, so effective contraception is required during treatment and for some time after stopping therapy. Breastfeeding is not recommended while taking nilotinib.
When taken correctly and monitored closely, nilotinib is highly effective in controlling chronic myeloid leukaemia, helping many patients achieve long-term remission and maintain a good quality of life. Regular follow-up, blood tests, and strict adherence to dosing instructions are essential for safe and successful treatment.
