Irinotecan is a chemotherapy medicine used mainly to treat cancers of the colon and rectum, especially advanced or metastatic colorectal cancer. It may also be used in other cancers depending on treatment protocols. Irinotecan belongs to a group of medicines called topoisomerase I inhibitors, which work by interfering with the way cancer cells copy and repair their DNA.
Irinotecan works by blocking an enzyme called topoisomerase I, which is essential for DNA replication. When this enzyme is blocked, the DNA inside cancer cells becomes damaged during cell division. This damage prevents the cancer cells from multiplying and eventually causes them to die. Because cancer cells divide more rapidly than normal cells, they are more sensitive to the effects of irinotecan.
Irinotecan is given as an intravenous infusion, usually in a hospital or oncology clinic. It is commonly given every one or two weeks, often in combination with other chemotherapy medicines such as fluorouracil and leucovorin. The exact dosing schedule depends on the treatment plan, the type of cancer, and how well the patient tolerates the medicine. Blood tests are done before each cycle to ensure it is safe to continue treatment.
One of the most important and well-known side effects of irinotecan is diarrhoea. There are two types. Early-onset diarrhoea can occur during or shortly after the infusion and may be accompanied by symptoms such as sweating, stomach cramps, runny nose, increased saliva, or watery eyes. This type is caused by stimulation of the nervous system and is usually treated quickly with medication such as atropine. Late-onset diarrhoea occurs more than 24 hours after treatment and can be severe and prolonged. This type requires prompt treatment with anti-diarrhoeal medicines such as loperamide and careful hydration to prevent dehydration.
Irinotecan can also cause suppression of the bone marrow. This can result in low white blood cell counts, increasing the risk of infections; low red blood cells, leading to fatigue and shortness of breath; and low platelets, which may cause bruising or bleeding. Because of this, regular blood tests are essential, and any signs of infection such as fever, chills, or sore throat should be reported immediately.
Other common side effects include nausea, vomiting, loss of appetite, fatigue, weakness, abdominal pain, hair thinning or hair loss, and mouth sores. These effects vary from person to person and are often manageable with supportive medications.
Irinotecan can also affect liver function, so liver enzymes are monitored during treatment. In some patients, it may cause electrolyte imbalances or dehydration, especially if diarrhoea is severe or prolonged.
In rare cases, irinotecan can cause serious complications such as severe infections, bowel inflammation, or life-threatening dehydration. Immediate medical attention is required if there is persistent diarrhoea, inability to drink fluids, dizziness, confusion, or reduced urine output.
Irinotecan may interact with other medications, including certain antibiotics, antifungal drugs, seizure medications, and herbal products such as St John?s wort. These interactions can affect how the drug is processed in the body and may increase side effects or reduce effectiveness. Patients should inform their healthcare provider about all medications and supplements they are taking.
This medicine can harm an unborn baby and should not be used during pregnancy. Effective contraception is required during treatment and for a period after completion. Breastfeeding is not recommended while receiving irinotecan.
Irinotecan is a powerful chemotherapy medicine that plays an important role in treating colorectal and other cancers. With careful monitoring, early management of side effects, and good communication with the healthcare team, many patients are able to tolerate treatment and gain meaningful benefit from therapy.
