Irinotecan is a chemotherapy medicine used mainly to treat cancers of the colon and rectum, especially advanced or metastatic colorectal cancer. It may also be used in other cancers depending on treatment protocols. Irinotecan belongs to a group of medicines known as topoisomerase I inhibitors, which interfere with the way cancer cells divide and multiply.
Irinotecan works by blocking an enzyme called topoisomerase I. This enzyme normally helps relax DNA strands during cell division. When irinotecan blocks this enzyme, the DNA becomes damaged and the cancer cell can no longer divide properly, leading to cell death. Cancer cells are more sensitive to this effect because they divide more rapidly than normal cells.
Irinotecan is given as an intravenous infusion, usually in a hospital or oncology clinic. It is often used in combination with other chemotherapy medicines such as fluorouracil and leucovorin. The dosing schedule depends on the treatment plan, cancer type, and how well the patient tolerates the medicine. Treatment is usually given in cycles, with rest periods in between to allow the body to recover.
One of the most important side effects of irinotecan is diarrhoea, which can occur in two different forms. Early-onset diarrhoea may occur during or shortly after the infusion and is often accompanied by symptoms such as sweating, abdominal cramping, watery eyes, or flushing. This type is caused by a cholinergic effect and can usually be treated quickly with medication such as atropine. Late-onset diarrhoea occurs more than 24 hours after treatment and can be severe and prolonged. This requires prompt treatment with antidiarrhoeal medicines such as loperamide and close medical monitoring to prevent dehydration.
Irinotecan can also cause suppression of the bone marrow, leading to low white blood cell counts, low red blood cell counts, and low platelets. This increases the risk of infections, anaemia, and bleeding. Blood tests are done regularly to monitor these levels, and treatment may be delayed or adjusted if counts are too low.
Other common side effects include nausea, vomiting, loss of appetite, fatigue, hair thinning or hair loss, abdominal pain, and weakness. Mouth sores may also occur. These effects vary from person to person and are often manageable with supportive care.
More serious side effects include severe infections, dehydration, electrolyte imbalances, and liver function abnormalities. Patients should contact their healthcare provider immediately if they experience fever, severe diarrhoea, dizziness, reduced urination, or signs of infection.
Irinotecan can interact with other medicines, including certain antibiotics, antifungal medications, seizure medicines, and herbal products such as St John?s wort. These interactions can affect how the drug is broken down in the body and increase side effects or reduce effectiveness. Patients should inform their healthcare team of all medicines they are taking.
This medicine can harm an unborn baby and should not be used during pregnancy. Effective contraception is required during treatment and for a period afterward. Breastfeeding is not recommended while receiving irinotecan.
Irinotecan requires careful monitoring and close communication with the healthcare team. When used correctly, it is an effective treatment that can slow disease progression, reduce tumour size, and improve symptoms in people with certain cancers.
