Gemcitabine is a chemotherapy medicine used to treat several types of cancer, including pancreatic cancer, lung cancer, bladder cancer, breast cancer, and ovarian cancer. Gemcitabine is often used alone or in combination with other chemotherapy medicines. If you have been prescribed gemcitabine, it means your oncology team has selected gemcitabine because it is appropriate for your cancer type and stage.
Gemcitabine works by interfering with how cancer cells make DNA. Cancer cells need to copy their DNA in order to grow and divide. Gemcitabine becomes incorporated into the DNA of cancer cells and also blocks enzymes needed for DNA production. This stops cancer cells from multiplying and leads to cancer cell death. Because cancer cells divide more rapidly than most normal cells, they are more affected by gemcitabine, but some healthy cells are also affected, which explains many of the side effects.
Gemcitabine is given by intravenous infusion into a vein. Gemcitabine is administered in a hospital or chemotherapy clinic by trained healthcare professionals. You will not take gemcitabine at home. Gemcitabine is usually given on specific days within a treatment cycle, such as once weekly for several weeks followed by a rest period. Each infusion usually takes about 30 minutes, although you may be at the clinic longer for monitoring and other medicines.
Now let?s talk about what to expect while receiving gemcitabine.
One of the most important effects of gemcitabine is lowering of blood counts. Gemcitabine can reduce white blood cells, red blood cells, and platelets. Low white blood cells increase the risk of infection, low red blood cells can cause fatigue and shortness of breath, and low platelets increase the risk of bruising or bleeding. Blood tests are done regularly. Seek urgent medical advice if you develop fever, chills, sore throat, unusual bruising, bleeding, or extreme tiredness.
Fatigue is very common with gemcitabine. Fatigue may be mild at first but can build up over time. This can be related to gemcitabine itself, low blood counts, or the cancer. Rest when needed and pace your activities.
Flu-like symptoms are common with gemcitabine, especially within a day or two after the infusion. These may include fever, chills, muscle aches, headache, or general discomfort. These symptoms are usually temporary. Any fever should still be reported, as infection must be ruled out.
Nausea and vomiting can occur with gemcitabine, but these are usually mild to moderate and often well controlled with anti-nausea medicines. Let your healthcare team know if nausea is not controlled.
Gemcitabine can cause shortness of breath or cough. Mild breathlessness can occur, but new or worsening shortness of breath, chest pain, or persistent cough should be reported immediately, as gemcitabine can rarely affect the lungs.
Gemcitabine can affect liver function. Blood tests are done to monitor this. Seek medical advice if you notice yellowing of the skin or eyes, dark urine, or persistent nausea.
Swelling of the hands, feet, or face can occur with gemcitabine due to fluid retention. Report swelling or sudden weight gain.
Skin reactions such as rash, itching, or redness may occur. These are usually mild but should be reported if severe or widespread.
Gemcitabine can cause hair thinning, although complete hair loss is less common than with some other chemotherapy medicines.
Gemcitabine can increase sensitivity to sunlight. Use sunscreen and protective clothing when outdoors.
Rarely, gemcitabine can affect the kidneys or cause blood clotting problems. Blood tests help monitor this. Report reduced urine output, unusual bruising, severe headache, or confusion.
Gemcitabine increases the risk of infection. Avoid close contact with people who are unwell, practise good hand hygiene, and seek medical advice promptly if you feel unwell.
Gemcitabine should not be used during pregnancy, as it can harm an unborn baby. Effective contraception is required during gemcitabine treatment and for the period advised after treatment ends. Breastfeeding is not recommended during gemcitabine therapy.
Gemcitabine can interact with other medicines, including other chemotherapy drugs and some vaccines. Always inform your pharmacist and healthcare team about all medicines and supplements you are taking.
