Gefitinib is an oral anticancer medicine used mainly to treat non-small cell lung cancer, particularly in patients whose cancer has specific genetic changes known as epidermal growth factor receptor (EGFR) mutations. It belongs to a group of medicines called tyrosine kinase inhibitors. These medicines work by blocking signals that tell cancer cells to grow, divide, and survive.
Gefitinib works by targeting the EGFR protein found on the surface of certain cancer cells. In some lung cancers, this receptor is overactive due to genetic mutations, causing uncontrolled cell growth. Gefitinib blocks the activity of this receptor, slowing or stopping the growth of cancer cells and helping to control the disease. Because of this targeted action, gefitinib is most effective in patients whose tumours test positive for EGFR mutations.
Gefitinib is taken by mouth, usually once daily, with or without food. It should be taken at the same time each day to maintain stable levels in the body. The tablet should be swallowed whole with water and not crushed or chewed. Treatment is usually long term and continues as long as the cancer responds and side effects remain manageable.
Many patients experience improvement in symptoms such as cough, shortness of breath, or chest discomfort within weeks of starting treatment. Gefitinib does not cure cancer, but it can significantly slow disease progression and improve quality of life in people with EGFR-positive lung cancer.
Common side effects include skin reactions such as acne-like rash, dry skin, itching, and nail changes. Diarrhoea is also very common and can usually be managed with fluids and anti-diarrhoeal medication. Other possible side effects include nausea, loss of appetite, fatigue, and mild mouth sores. These effects are often mild to moderate and may improve with time.
More serious side effects can occur, although they are less common. One of the most important is inflammation of the lungs, known as interstitial lung disease. This can cause symptoms such as shortness of breath, cough, or fever and can be life-threatening if not treated promptly. Any new or worsening breathing problems should be reported immediately.
Gefitinib can also affect liver function. Blood tests are regularly performed to monitor liver enzymes, especially during the first few months of treatment. Symptoms such as yellowing of the skin or eyes, dark urine, or severe fatigue should be reported immediately.
Because gefitinib is processed in the liver, it can interact with other medications. Certain antibiotics, antifungal medicines, seizure medications, and herbal supplements such as St John?s wort can reduce its effectiveness or increase side effects. Medicines that reduce stomach acid, such as proton pump inhibitors, may also reduce absorption and should be discussed with a healthcare provider.
Gefitinib should not be used during pregnancy, as it may harm the unborn baby. Effective contraception is required during treatment and for a period after stopping the medication. Breastfeeding is not recommended while taking gefitinib.
Regular follow-up appointments are essential to monitor response to treatment, manage side effects, and adjust therapy if needed. Blood tests and sometimes imaging studies are used to assess how well the cancer is responding.
When used appropriately in patients with EGFR-positive lung cancer, gefitinib can significantly slow disease progression, improve symptoms, and enhance quality of life. With careful monitoring and adherence to treatment, many patients benefit from long-term disease control.
