I know starting a medicine like erlotinib can bring up a lot of questions, and it is completely normal to want clarity on what erlotinib does, how to take it correctly and what to expect along the way. My goal is to make sure you feel supported, informed and confident throughout your erlotinib treatment.
Erlotinib is a targeted cancer medicine used for certain types of lung cancer and, in some cases, pancreatic cancer. Your doctor has prescribed erlotinib because your cancer cells have specific features that make them sensitive to this type of treatment. Erlotinib works by blocking a signal inside cancer cells called the epidermal growth factor receptor, or EGFR. Many cancer cells rely on this pathway to grow and divide. When erlotinib blocks that signal, the cancer cells slow down or die off, helping control the disease.
Erlotinib is taken by mouth once daily. It is very important to take erlotinib on an empty stomach ? at least one hour before eating or two hours after eating. Food increases the absorption of erlotinib, which can lead to higher levels in the body and increase the chance of side effects. Try to take erlotinib around the same time every day to keep your levels steady. If you ever miss a dose, do not double up. Just take your next dose at the usual time.
Now, let?s talk about what to expect while taking erlotinib.
One of the most common effects of erlotinib is a skin reaction. Many patients develop a rash, often on the face, chest or upper body. The rash may look like acne or small bumps and can feel tender or itchy. This rash is actually a sign that erlotinib is working on its target, but it can be uncomfortable. Gentle skincare, fragrance-free moisturisers and daily sunscreen can help a great deal. If the rash becomes painful or widespread, please let your doctor know ? they can prescribe creams or tablets to help manage it.
Dry skin, cracking around the fingertips and sensitivity to sunlight are also common with erlotinib. Using moisturisers regularly and protecting your skin from the sun will reduce discomfort.
Erlotinib can also affect the digestive system. Diarrhoea is a frequent side effect. Staying hydrated, eating smaller meals and avoiding very greasy or spicy foods can help. If diarrhoea becomes severe or you cannot maintain hydration, you must contact your doctor, as they may provide antidiarrhoeal medicines or adjust your treatment.
Some people experience nausea, loss of appetite or mild stomach discomfort with erlotinib. These can often be managed by adjusting meal patterns or using medicines prescribed by your doctor if needed. If you find your appetite remains low for several days, please reach out so your care team can support you.
Another important point with erlotinib is lung health. Very rarely, erlotinib can cause inflammation in the lungs. If you ever notice new or worsening shortness of breath, a dry persistent cough, fever or chest discomfort, please seek medical attention promptly. These symptoms must be reviewed right away.
Erlotinib can also affect the eyes, causing dryness, irritation or increased tearing. If you experience eye pain, vision changes or severe discomfort, please see your doctor or an eye specialist promptly.
There are a few medicines and supplements that cannot be taken with erlotinib because they change the acidity in the stomach and affect how well erlotinib is absorbed. Examples include some heartburn or reflux medicines. Always check with me before starting anything new, even non-prescription or herbal products.
Smoking significantly reduces the effectiveness of erlotinib. If you are currently smoking, I strongly encourage you to discuss support options with your healthcare team, as quitting can directly improve how well this treatment works for you.
Throughout your treatment, your doctor will arrange blood tests to monitor your liver function and overall response. You do not need to interpret these tests yourself ? they are simply part of keeping you safe on erlotinib.
Erlotinib is often taken for many months, and it is understandable to have days of worry or tiredness. Please remember that you are not going through this alone. Your oncology team and I as your pharmacist are here to support you through every part of your erlotinib journey.
If you ever feel unsure, if your symptoms change or if you need help managing side effects, please don?t hesitate to speak with me. My role is to keep you safe, informed and comfortable while you are on erlotinib.
