Bevacizumab is an important medicine used in the treatment of several cancers, including cancers of the bowel, lung, kidney, cervix, ovary and brain. Bevacizumab is also used in some non cancer conditions when abnormal blood vessel growth causes health problems. Beginning bevacizumab can feel overwhelming, and it is normal to want to understand what bevacizumab does, how bevacizumab works and what you might experience while receiving bevacizumab. My goal is to guide you through these details in a way that supports and reassures you.
Bevacizumab works by targeting a protein in the body called vascular endothelial growth factor, known as VEGF. VEGF is responsible for telling the body to grow new blood vessels. Many cancers produce large amounts of VEGF to build new blood vessels that supply the tumour with oxygen and nutrients. When bevacizumab attaches to VEGF, it prevents the tumour from forming new blood vessels. This slows tumour growth and can shrink the cancer or make it more responsive to chemotherapy. Because bevacizumab affects blood vessel development, bevacizumab is known as an anti angiogenic medicine.
Bevacizumab is given as an intravenous infusion in the clinic or hospital. The first bevacizumab infusion is usually given slowly while your vital signs are closely monitored. If you tolerate bevacizumab well, later infusions can be given more quickly. The schedule for bevacizumab depends on your diagnosis, but most people receive bevacizumab every two or three weeks. Bevacizumab is often combined with chemotherapy, but it can also be used alone after chemotherapy is completed to maintain cancer control.
Because bevacizumab works on blood vessels, it has specific side effects that are important to understand. One of the most important effects of bevacizumab is high blood pressure. Many patients develop a rise in blood pressure while receiving bevacizumab, and this often requires monitoring and sometimes starting a blood pressure medicine. It is important to check your blood pressure regularly at home or during clinic visits while receiving bevacizumab.
Bevacizumab can affect wound healing. Because bevacizumab slows new blood vessel formation, the body takes longer to repair tissues. For this reason, bevacizumab is usually stopped several weeks before planned surgery and restarted only after your doctor confirms that you have healed well. If you need dental extractions or any procedure that may involve bleeding or tissue repair, tell your care team if you are receiving bevacizumab.
Another important effect of bevacizumab is the risk of bleeding. This may appear as nosebleeds, gum bleeding or small bruises. In rare cases, bevacizumab can cause more serious bleeding in the lungs, stomach or brain. It is important to report coughing up blood, black stools, vomiting blood, severe headaches or sudden changes in vision or speech while on bevacizumab.
Bevacizumab can also increase the risk of blood clots. Pain, swelling, warmth or redness in the legs, or sudden chest pain and shortness of breath should be reported immediately, as they may indicate a clot. Although this is uncommon, it is a known risk of bevacizumab.
Some patients receiving bevacizumab may develop protein in the urine, which can be a sign of kidney strain. Your urine will be checked regularly while receiving bevacizumab to ensure your kidneys remain healthy. If swelling in the ankles or face develops, this should be mentioned to your team.
Bevacizumab may occasionally cause bowel perforation, particularly in people with underlying bowel disease or those who have had multiple abdominal surgeries. Severe abdominal pain, fever or persistent vomiting while on bevacizumab should be treated as urgent symptoms.
More common and less dangerous side effects of bevacizumab include fatigue, headaches, changes in taste, mild diarrhoea or dry skin. These can usually be managed with supportive care and tend to fluctuate from cycle to cycle.
Before starting bevacizumab, your medical team will ensure that your blood pressure is stable, your kidney function is adequate and there are no active infections. During treatment, regular blood pressure checks, urine tests and physical assessments help ensure bevacizumab remains safe for you. Because bevacizumab is not processed through the liver in the same way as many oral medicines, bevacizumab does not usually interact with most tablets. However, the chemotherapy used alongside bevacizumab may have its own interactions, and your team will monitor this.
Pregnancy must be avoided during bevacizumab treatment and for several months after finishing bevacizumab because bevacizumab can harm a developing baby. Your care team will discuss contraception options if needed.
Through all of this, please know that you are not alone while receiving bevacizumab. Many people find that bevacizumab becomes a stable part of their long-term treatment plan once they understand how it works and what to expect. Your doctors, nurses and I as your pharmacist will support you closely throughout bevacizumab therapy, helping you manage side effects, monitor your progress and stay informed at every step.
You are taking a meaningful step by beginning bevacizumab, and even on challenging days you are showing strength by continuing forward. If you ever feel unsure, have new symptoms or need help understanding any part of your bevacizumab treatment, please reach out. I am here with you throughout your entire bevacizumab journey.
