Bacillus Calmette-Gu?rin, commonly called BCG, is a live, weakened form of a bacterium related to tuberculosis. BCG is used in two very different clinical settings: as a vaccine to protect against severe forms of tuberculosis, and as an intravesical treatment for certain types of bladder cancer. If you have been prescribed or offered Bacillus Calmette-Gu?rin BCG, it is important to understand why BCG is being used in your case, how BCG works, how it is given, and what to expect after BCG treatment.
BCG works by stimulating the immune system. Rather than directly killing bacteria or cancer cells, Bacillus Calmette-Gu?rin activates your body?s immune response so that immune cells become more alert and aggressive in the area where BCG is administered. The way BCG is used and the effects you may experience depend on whether BCG is being given as a vaccine or as bladder cancer treatment.
When Bacillus Calmette-Gu?rin is used as a vaccine, it is usually given as a single injection into the skin, often in childhood. The BCG vaccine helps protect against severe forms of tuberculosis, particularly in children, such as TB meningitis. The BCG vaccine does not completely prevent tuberculosis infection, but it reduces the risk of serious complications. After BCG vaccination, it is common to develop a small sore at the injection site that may blister, crust and heal over several weeks, often leaving a small scar. This is a normal response and indicates that the immune system has reacted to the BCG.
When Bacillus Calmette-Gu?rin is used for bladder cancer, it is given directly into the bladder through a catheter. This is called intravesical BCG therapy. BCG is used for early-stage, non-muscle-invasive bladder cancer to reduce the risk of the cancer returning or progressing. BCG is not chemotherapy. Instead, BCG triggers a strong immune reaction inside the bladder that helps destroy cancer cells and prevent regrowth.
Intravesical BCG is usually given once a week for several weeks as an induction course, followed by maintenance treatments in some patients. During BCG bladder treatment, the liquid BCG is held in the bladder for about two hours before being passed in the urine. After BCG bladder instillation, it is very common to experience bladder-related symptoms. These include frequent urination, urgency, burning or discomfort when passing urine, and mild lower abdominal pain. These symptoms usually start within a few hours of BCG treatment and may last for one to three days. Drinking fluids after the recommended holding time can help flush the bladder and reduce discomfort.
You may also notice blood in the urine after BCG treatment. Small amounts of blood are common, especially during the first few treatments. However, heavy bleeding, large clots, or bleeding that does not improve should be reported promptly.
General flu-like symptoms are also common with Bacillus Calmette-Gu?rin. These include fever, chills, fatigue, muscle aches and headache. Mild symptoms are expected and usually settle within 24 to 48 hours. Paracetamol can be used if advised by your doctor. If fever is high, persistent, or worsening beyond 48 hours, medical review is required.
Because BCG is a live bacteria, there are important safety points to follow after bladder treatment. For the first several hours after passing urine, you may be instructed to disinfect the toilet with bleach and wash your hands thoroughly. This helps prevent BCG bacteria from spreading to others. Follow the exact instructions provided by your treatment team.
Serious side effects from Bacillus Calmette-Gu?rin are uncommon but important to recognise. Seek urgent medical attention if you develop high fever lasting more than 48 hours, severe weakness, night sweats, cough, shortness of breath, joint pain, skin rash, or signs of infection elsewhere in the body. These symptoms may indicate a rare but serious systemic BCG infection and must be treated promptly.
BCG should not be given to people with severely weakened immune systems, active infections, or significant bleeding from the bladder. Your doctor has assessed that BCG is appropriate for you. If you become unwell, start immunosuppressive medicines, or develop a new infection during BCG therapy, inform your healthcare team before the next dose.
