Azacitidine is a chemotherapy medicine used mainly to treat myelodysplastic syndromes (MDS), acute myeloid leukaemia (AML), and certain other bone marrow disorders. These conditions affect how the bone marrow produces blood cells, often leading to low blood counts, infections, bleeding, and fatigue. Azacitidine helps improve bone marrow function and can slow disease progression.
Azacitidine works by affecting how genes inside abnormal cells behave. It belongs to a class of medicines called hypomethylating agents. It works by restoring normal gene function in abnormal blood cells and by stopping cancer cells from growing and dividing. This helps the bone marrow produce healthier blood cells over time and reduces the number of abnormal cells.
Azacitidine is usually given as an injection under the skin or into a vein, depending on the treatment plan. It is commonly administered for 7 consecutive days every 28 days, although schedules may vary based on the patient?s condition and tolerance. Treatment is usually given in repeated cycles, and improvement may take several cycles to become noticeable.
Because azacitidine affects the bone marrow, blood counts often drop during the first few cycles before improvement occurs. This can cause low white blood cells, red blood cells, and platelets, increasing the risk of infection, tiredness, and bleeding. For this reason, regular blood tests are essential, and treatment may be delayed or adjusted if counts fall too low.
Common side effects include nausea, vomiting, diarrhoea, constipation, fatigue, fever, injection site reactions, and loss of appetite. These effects are usually manageable and tend to improve as treatment continues. Anti-nausea medicines are often given to help reduce discomfort.
Azacitidine can increase the risk of infections. Patients should report fever, chills, sore throat, cough, or any signs of infection immediately. Good hygiene and avoiding contact with sick individuals are important during treatment.
Other possible side effects include bruising or bleeding due to low platelets, shortness of breath, dizziness, and weakness. Injection site reactions such as redness, swelling, or pain are common but usually mild.
Azacitidine may affect liver and kidney function, so regular blood tests are done to monitor organ function. Patients with kidney or liver disease may need dose adjustments.
This medicine can harm an unborn baby and should not be used during pregnancy. Effective contraception is required during treatment and for some time afterward. Breastfeeding is not recommended while receiving azacitidine.
Treatment with azacitidine often requires patience, as benefits may not be seen immediately. Many patients need several cycles before improvements in blood counts or symptoms occur. Continuing treatment as advised is important to achieve the best response.
When used under specialist supervision, azacitidine can improve survival, reduce transfusion needs, and enhance quality of life for people with bone marrow disorders. Regular follow-up, blood monitoring, and communication with the healthcare team are essential for safe and effective treatment.
