Temozolomide is a chemotherapy medicine used mainly to treat certain brain cancers, including glioblastoma and other malignant gliomas. Temozolomide may be used on its own or together with radiotherapy, and sometimes as long-term treatment to help control the disease. If you have been prescribed temozolomide, it means your specialist team has selected temozolomide because it can cross into the brain and is effective against your type of tumour.
Temozolomide works by damaging the DNA inside cancer cells. Cancer cells need to repair and copy their DNA in order to grow and survive. Temozolomide adds chemical changes to the DNA that prevent cancer cells from repairing themselves properly. As a result, cancer cells stop dividing and die. Because cancer cells divide more rapidly than most normal cells, they are more affected by temozolomide, although some normal cells are also affected, which explains many of the side effects.
Temozolomide is taken by mouth as capsules. Temozolomide is usually taken once daily on specific days depending on your treatment schedule. Some people take temozolomide daily for several weeks with radiotherapy, followed by monthly cycles where temozolomide is taken for a few days each month. Your doctor has chosen the exact dose and schedule based on your condition.
Temozolomide capsules should be swallowed whole with a full glass of water. Do not open, crush, or chew temozolomide capsules. Temozolomide is best taken on an empty stomach, usually at bedtime, to reduce nausea. Wash your hands before and after handling temozolomide. If a capsule is damaged, avoid contact with the contents and inform your pharmacist.
Now let?s talk about what to expect while taking temozolomide.
Nausea and vomiting are common with temozolomide, especially when starting treatment. Anti-nausea medicines are often prescribed and should be taken exactly as directed. Taking temozolomide at night and on an empty stomach helps reduce nausea. Report ongoing or severe vomiting so treatment can be adjusted.
One of the most important effects of temozolomide is lowering of blood counts. Temozolomide can reduce white blood cells and platelets. Low white blood cells increase the risk of infection, and low platelets increase the risk of bruising or bleeding. Blood tests are done regularly, sometimes weekly. Seek medical advice urgently if you develop fever, chills, sore throat, unusual bruising, bleeding, black stools, or pinpoint red spots on the skin.
Because temozolomide lowers immunity, certain infections can become serious. In some cases, additional medicines are prescribed to prevent specific lung infections while taking temozolomide, especially during combined treatment with radiotherapy. Take any preventive medicines exactly as prescribed.
Fatigue is very common with temozolomide. Fatigue may be mild or significant and can be related to the medicine, the cancer, or radiotherapy. Rest when needed and pace daily activities.
Constipation or diarrhoea may occur with temozolomide. Staying hydrated and reporting persistent bowel changes early can help prevent complications.
Loss of appetite and taste changes are common. Eating small, frequent meals may help.
Hair thinning may occur with temozolomide, particularly when used with radiotherapy, although complete hair loss is less common than with some other chemotherapy medicines.
Temozolomide can cause headache or dizziness. Report severe or persistent headaches, especially if associated with vomiting or vision changes.
Temozolomide can affect liver function. Blood tests help monitor this. Seek medical advice if you notice yellowing of the skin or eyes, dark urine, or persistent abdominal pain.
Temozolomide can increase the risk of bruising or bleeding. Avoid activities with a high risk of injury and use caution with sharp objects.
Temozolomide should not be used during pregnancy, as it can harm an unborn baby. Effective contraception is required during temozolomide treatment and for the period advised after stopping treatment, for both men and women. Breastfeeding is not recommended during temozolomide therapy.
Temozolomide can interact with other medicines that suppress the immune system or affect blood counts. Always inform your pharmacist and healthcare team about all medicines and supplements you are taking.
If you miss a dose of temozolomide, contact your doctor or pharmacist for advice. Do not take an extra dose unless instructed. If vomiting occurs after taking temozolomide, do not repeat the dose unless told to do so.
Do not stop temozolomide unless your doctor tells you to. Temozolomide is most effective when taken exactly as prescribed according to the treatment schedule.
As your pharmacist, my role is to help you understand how to take temozolomide correctly, recognise expected and serious side effects, and know when to seek medical advice. I do not adjust temozolomide doses or manage oncology decisions, but I support safe and informed use through counselling.
Temozolomide is an important treatment for certain brain cancers. Understanding how temozolomide works and what to expect helps you take temozolomide safely and confidently as part of your overall treatment plan.
