Bendamustine is a chemotherapy medicine used mainly to treat certain blood cancers, especially chronic lymphocytic leukaemia, non-Hodgkin lymphoma, and some slow-growing lymphomas. It belongs to a group of medicines known as alkylating agents, but it also has properties similar to purine analogues, which makes it effective even in cancers that no longer respond well to other treatments.
Bendamustine works by damaging the DNA inside cancer cells. This damage prevents the cells from dividing and multiplying, leading to cell death. Because cancer cells divide more rapidly than normal cells, they are more affected by this damage. However, some normal cells can also be affected, which explains many of the side effects associated with treatment.
Bendamustine is given by intravenous infusion in a hospital or specialist treatment centre. It is usually given on one or two consecutive days in a treatment cycle, with cycles repeated every few weeks depending on the condition being treated and how well the patient tolerates the medication. The dose is carefully calculated based on body size, kidney function, and overall health.
One of the most important effects of bendamustine is suppression of the bone marrow. This can lead to low white blood cell counts, increasing the risk of infections; low red blood cell counts, causing tiredness and shortness of breath; and low platelet counts, which can increase the risk of bruising or bleeding. Because of this, regular blood tests are essential before and during treatment.
Common side effects include fatigue, nausea, vomiting, loss of appetite, fever, headache, and skin reactions such as rash or itching. Hair thinning may occur, although complete hair loss is less common compared with some other chemotherapy drugs. Some people may also experience mild diarrhoea or constipation.
More serious side effects can include infections, infusion reactions, allergic reactions, and severe skin reactions. Symptoms such as fever, chills, difficulty breathing, rash, or swelling during or after infusion should be reported immediately. Bendamustine can also increase the risk of tumour lysis syndrome, especially in patients with a high tumour burden, so hydration and monitoring are important during early treatment.
Bendamustine may affect the liver and kidneys, so blood tests are regularly performed to monitor organ function. Dose adjustments may be needed in patients with existing liver or kidney disease.
This medicine can weaken the immune system, so patients should avoid close contact with people who are sick and should inform their healthcare provider if they develop signs of infection such as fever, sore throat, or persistent cough. Live vaccines should be avoided during treatment.
Bendamustine can harm an unborn baby and should not be used during pregnancy. Effective contraception is required during treatment and for a period afterward. Breastfeeding is not recommended while receiving bendamustine.
Although bendamustine is a powerful chemotherapy medicine, it is generally well tolerated when given under proper medical supervision. It has proven to be highly effective in treating certain blood cancers and can significantly improve disease control and quality of life when used appropriately.
